O Lord, Thou dwellest on high and lookest upon the humble; as of old Thou didst receive the prayer and tears of Hezekiah, and didst raise him up from his sickbed and prolong his life by fifteen years; as through the prophet Elisha Thou didst cleanse Naaman the Syrian of leprosy, and didst raise up the son of the woman of Shunem; and as Thou didst deliver Peter's mother-in-law from fever, and by Thy word didst heal lepers, and by the touch of Thy garment didst heal the woman who was sick twelve years with an issue of blood; and for the faith of the centurion didst heal his servant; and didst raise the widow's only son from the dead and give him back alive and well to his mother; and didst free the daughter of the Canaanite woman from the fierce demon; and having anointed with clay the eyes of one blind from birth, didst restore his sight; and didst raise Thy beloved friend Lazarus on the fourth day and give him back to his sisters; as Thou once gavest power to Thy Apostles to heal the sick and to cleanse lepers; as Thou didst justify the publicans, and, nailed to the Cross, didst promise Paradise to the thief, Thou Who takest upon Thyself the sins of the world; as through Thy chief Apostles Peter and John Thou didst heal the lame man at the Beautiful Gate, and raise Tabitha from the dead, and through Paul didst raise Eutychus, and didst keep Paul's hand unharmed from the viper's bite, and through Paul Thy Apostle, by Thy mighty hand, didst heal the father of Publius of fever; so also now, by Thine ineffable love for mankind and Thy accustomed mercy, heal Thy servant (name) of his (or her) spiritual sufferings. We pray Thee and beseech Thee, forgive Thy servant (or handmaiden) all the transgressions for which he entreats Thee, and heal him of his passions, cleanse him of every impurity of body and soul, quench with the dew of Thy mercy the fire within him; forgive him if he has transgressed any of Thy commandments, as one who bears flesh and lives in the flesh, if he has sinned before Thee by day or by night, willingly or unwillingly, knowingly or unknowingly, or if his mind was harmed by the sight of the eyes or the hearing of the ears or by smell or by anything else, and thus he sinned in body and in spirit, and estranged himself from Thy will; but do Thou Thyself, O Lord, as good and loving to mankind, raise him up from his grievous sickness and suffering.
O Master and Lord, hear us sinners now and whenever we call upon Thee; look upon us as one who is good, and remember not his former transgressions, and deliver him (or her) and us from eternal torment, and cleanse him and us from every evil intention, and let his mouth magnify Thee, and his lips glorify Thee, and strengthen his hands to do Thy commandments, and direct his feet in Thy way, and confirm all his members and his mind through the prayers of Thy most pure Mother and of all Thy Saints.
For Thou art a merciful and loving God, and unto Thee we send up glory, with Thy beginningless Father, and Thy good and life-giving Spirit, now and ever and unto the ages of ages. Amen.
